I grant Penn Foster Group the right to use my written comments, my name, my photograph, videos of me, and any other submissions I make for promotion and advertising of all associated brands/schools (e.g., Penn Foster, Ashworth College, James Madison High School, NYIAD, NYIP, and Career Step etc.). Such use may include all or part of these items, they may be combined with other materials, and Penn Foster Group is permitted to make reasonable revisions to them. I understand I am not entitled to any compensation for use, reproduction, or republication of these statements. I hereby affirm that all statements, comments, and other information provided by me are true and reflect actual events. Further, I fully release Penn Foster Group from all liability that could arise from the release granted. I agree that Penn Foster Group has no obligation to publish, use, or advise me as to whether or when the school(s) will publish or use my comments, name, or photographs. Penn Foster Group shall have the right to edit my statements, video, and photo for editorial purposes in Penn Foster Group’s discretion. 

I authorize Penn Foster Group to share with vendors my phone number, email, address, and other contact information on file with Penn Foster Group so that they can reach me by those means to provide me with copies of photographs and videos.  These communications may be by automated means. I understand that the photographers may sell licenses for the photographs and videos to me in accordance with terms and conditions established by the photographer, provided that nothing in those terms shall limit the usage rights I am providing to Penn Foster. I also understand that Penn Foster Group is not a party to any transaction between me and the photographer.

I agree to the following behavioral standards: The possession and/or use of drugs and/or alcohol at the arena is forbidden. Possession of weapons or incendiary devices of any kind is prohibited. Contraband is confiscated upon discovery; expulsion follows immediately. Smoking is prohibited inside the arena. PFG deems unacceptable any verbal or physical conduct that demeans others because of their race, gender, ethnic background, religion, or sexual orientation.

I understand that I am registering to attend the graduation ceremony being held on Saturday, August 3, 2024 at the Gas South Arena in Duluth, GA. I understand that graduates must have met all academic and financial obligations for their program in order to attend the graduation ceremony. I understand that my registration payment is nonrefundable. I understand that guest tickets, including those for parents/guardians, and regalia (cap, gown, and tassel) will need to be purchased separately.

I give my consent to Penn Foster Group staff to arrange for referral, transportation, and administration of emergency or urgent care, procedures, and treatment by local licensed medical facilities which are deemed necessary and in the best interest of the minor participant. This includes consent for Penn Foster Group's staff to administer such necessary care, procedures, and treatment. Prior to prescribing treatment or referring your child / ward to an outside medical provider, Penn Foster Group will make every reasonable attempt to contact a parent or guardian.

As long as the medical treatment is considered necessary in the situation and is in accordance with generally accepted standard of medical practice for the particular type of injury or illness involved, I impose no specific limitations or prohibitions regarding treatment other than those that I inform you about.  If you need to tell us about medical conditions, including allergies, please email our Grad Support team.
